What will you be doing in this role?
The Outpatient per diem Clinical Social Worker with Obstetrics and Gynecology and the Reproductive Psychology Program will be responsible for providing outreach to patients at PMAD risk or have experienced a perinatal loss as well follow up with clinical referrals and community inquires. The Clinical Social Worker is also responsible for brief assessment, psycho education, specialized care navigation/resource linkage, risk assessment, and group facilitation for pregnant/postpartum women. The position requires experience and facility in brief therapy interventions, counseling, crisis intervention, problem-solving and conflict resolution, patient and family management, transdisciplinary collaboration, bio psychosocial assessments, navigation, education, advocacy, and community resource linkages. Opportunities available to engage in current and future research studies and publications, though not required.
Complies with regulatory requirements (i.e. BBS) appropriate for position.
Respond in a timely manner to community inquires and referrals initiated by various members of the medical teams.
Manage and monitor distress screening tools and apply these to triage patient needs and refer them to the appropriate resources.
Connect patients with appropriate resources (whether at Cedars or in the community), including insurance-contracted perinatal therapists, therapy/support groups, perinatal intensive outpatient programs, intimate partner violence/sexual violence resources, lactation consultant services, financial support, Public and Private Benefits, etc.
Provide brief psychotherapy under the supervision of a licensed clinician.
Participate in and manage the proper reporting of suspected abuse/neglect and other government-related reporting such as Child Protective Services, Adult Protective Services, Office of the Public Guardian, Long-Term Care Ombudsman, Community Care Licensing, and law enforcement.
Advocate for patient rights and self-determination in the medical system.
Complete comprehensive documentation in the patient?s electronic medical record, consistent with legal and organizational requirements.
Collaborate with the transdisciplinary treatment team of physicians, nurses, social workers, psychologists and medical staff to ensure integration of care.
Attend monthly inter/multidisciplinary team meetings.
Perform other relevant duties in the scope of social work as assigned.
Qualifications
Education:
Master?s degree in Social Work from an accredited college.
License/Certification:
Maintain active registration in good standing with State of California, Board of Behavioral Science required.
Perinatal Mental Health Certification (PMH-C) preferred.
Experience:
1-2 years clinical social work experience, including a minimum of one-year experience in a healthcare setting preferred.
Experience with perinatal patient population preferred.
Possess excellent assessment skills and ability to implement brief behavioral interventions.
Excellent writing and communication skills.
Ability to exercise objectivity and good judgment relating to difficult and emotionally charged situations and the ability to interface effectively with patients, families, providers, management, and employees of the organization as well as facilities and agencies in the community, all while maintaining appropriate boundaries.
Possess knowledge of community resources, Public and Private Benefits.
Experience and demonstrated skill in working with a transdisciplinary team.
