Nurse - Clinical Doc Specialist
Delivers fundamental knowledge-based care to the program while developing technical skills. Assesses, plans, implements, and evaluates delivery of care based on age specific components. Collaborates with members of the interdisciplinary team and leading to process improvement initiatives. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
