The Staff Training in Assisted Living Residencies in VA (STAR-VA)/Behavioral Recovery Outreach (BRO) Team Coordinator is responsible for program development and implementation. The incumbent will manage the daily operation of the program, develop policies and procedures for program operation, and prepare reports and analyses for facility, Veterans Integrated Service Network (VISN), and national use. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Minimum of 2 years successful nursing practice; Demonstrated ability to lead multiple, complex programs and initiatives; Experience in leading change; Master's Degree in Nursing or comparable degree; Experience in the care of complex dementia patients; Evidence of self-directed and independent nursing practice; BLS Certification; Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. The Incumbent manages and controls the use of data related to the program regarding day to day functioning, outcomes and program evaluation. S/he is responsible for timely completion of all program evaluations and documentation required by the Community Living Center (CLC), Nursing Service Line, Behavioral Health Service Line, Medical Center, VISN 20, and VACO and is responsible for assuring compliance with appropriate accrediting/regulatory bodies such as Joint Commission, OIG and any future accrediting bodies (e.g., CARF). S/he will work in conjunction with CLC, Nursing and BH leadership, the VISN 20 Behavioral Recovery Programs Manager, and other facility and VISN service lines, to plan, organize, implement, and direct operations for patient care and personnel activities for the service. S/he will be responsible for facilitating and collaborating with internal and external stakeholders, which may include the contract nursing home program, mental health and primary care service line staff, CLC, inpatient, and community housing resources. The Incumbent's clinical work will be primarily within the STAR-VA/BRO Team. S/he will serve STAR-VA/BRO patients within the CLC/Acute units, be a resource consultant for complex, difficult to discharge patients hospitalized at the Boise VA Medical Center (BVAMC), and serve as a liaison to those patients served in supportive community settings (e.g. community nursing homes, assisted living settings, medical foster homes, group homes, etc.). The Incumbent will be responsible for providing clinical care, working with the interprofessional teams to assist in the creation and implementation of effective behavioral treatment plans, and provide recommendations to medical staff, community partners, and family members. The STAR-VA/BRO Team Coordinator assists in behavior plan development, guiding formulation of behavioral assessment using information obtained from staff partners, and coordinating the implementation of behavioral interventions. The Incumbent may act as a consultant for problems of distressed behavior and provide education/training for staff on effective behavioral strategies in other staff partners inpatient VA settings outside of the CLC. The Incumbent will provide ongoing care transition services to STAR-VA/BRO Team Veterans post-discharge, collaborating with all relevant stakeholders to help ensure the Veterans' success in the community. The Veteran population served by the STAR-VA/BRO Teams commonly exhibit behavioral disturbances and dementia-related behaviors. They commonly have complex psychosocial circumstances, ethically challenging situations, and limited psychosocial supports and/or limited financial resources. The incumbent is capable of practicing at the advanced practice level and has specialized knowledge of practice typically related to a particular diagnosis or population. The incumbant also has excellent interpersonal and communication skills, evidence of dependability in work related environment, and is able to function autonomously as well as active member of interdisciplinary team.
