The Northeast Ohio VA Healthcare System is recruiting for several Licensed Practical Nurse positions to function within Patient Care Services at the Louis Stokes VA Medical Center. Work Schedule: Variable shift (rotating all 3 tours), days/evening AND nights, rotating weekends and holidays. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Must be a graduate of school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing. Must possess a full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. Preferred Experience: Minimum 1 year experience preferred. IV medication administration experience preferred. Grade Determinations: GS-6: Completion of at least 1 year of qualifying experience at the GS-5 level or equivalent fully meeting all performance requirements for the GS-5 LPN/LVN. Applicants must meet the following: Technical proficiency in initiating, performing and completing assigned duties in providing care to variable patient populations. Ability to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant] patient information. Ability to observe, identify and respond to the patient's needs for care. Ability to prepare and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures. Knowledge and ability to recognize urgent or emergent patient care situations. Knowledge of handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during, and following the procedure. Physical Requirements: Applicant must possess the ability to do the following: heavy lifting and carrying, 45 pounds and over; pulling, pushing and reaching above the shoulder; walking, standing, kneeling and repeated bending. Ability to distinguish basic colors, correctable vision, and emotional/mental stability. ["Coral Cove (CT2) is a 37-bed short stay rehab neighborhood. The practical nurse collaborates with an interdisciplinary team to provide comprehensive care to Veterans that are completing rehab with the goal to return to their prior functional status after an acute medical stay. Admiral's Point (3B) is a 14 bed post-acute neighborhood with a respiratory focus. The practical nurse collaborates with an interdisciplinary team to provide comprehensive care to the Veterans that require more respiratory focused care then what can be supported in the home environment. Waterside Way (CT3) & Emerald Woods (CT4) are both 37 bed short stay rehab and skilled neighborhoods. The practical nurse utilizes the nursing process to provide comprehensive care to Veterans that are admitted for rehab, IV antibiotic management, and simple to complex wound care. The nurse will identify barriers to rehab or treatment plan and will collaborate with an interdisciplinary team to resolve these issues promote a safe discharge to the Veterans preferred location. Soaring Heights (CT5) is a 37-bed secure cognitive impairment/dementia care neighborhood. The practical nurse provides comprehensive holistic care to veterans that are admitted to the floor due to respite, placement needs, short term rehab or in need of medical care that cannot be safely given at home. The nurse will collaborate with the interdisciplinary team to facilitate a safe and appropriate discharge. Blind Rehab (WCTA) is a 15 Bed certified rehabilitation center for Veterans with various degrees of visual impairment. The practical nurse works with the interdisciplinary team to work with the Veteran while they receive several weeks of therapies with the goal to return home to independent living. Heroes Harbor Hospice is a 12 bed neighborhood that provides services to Veterans enrolled in the VA NEOHS who have been diagnosed with a life-limiting illness for which curative treatment is no longer appropriate, that would benefit from an inpatient interdisciplinary team, providing patient centered care with the goal to achieve the best quality of life through relief of suffering, control of symptoms and maintenance of functional capacity while remaining sensitive to personal, cultural and religious values, beliefs and practices. This also includes bereavement care for the Veteran's family. SCI Long Term Care is a home-like environment where the Veteran stays the duration of his/her life. The practical nurse collaborates with an interdisciplinary team to provide comprehensive care to the Veterans that require a more in-depth clinical care of the spinal cord injury or neurologically impaired condition than what can be supported in the home environment. The nurses and care team strive to respect every Veterans preference and offer individualized care. Many veterans express a family atmosphere between staff and other residents. The incumbent will provide prescribed medical treatment and personal care services to ill and injured veterans. Duties include: Providing direct patient care to Veterans with minimal supervision. Preparing and administering medications, including vaccinations. Performs treatments and procedures within the scope of license and facility policy and procedures. Seeks assistance from appropriate resource person as needed. Actively communicates and listens clearly in writing and in speaking. Accurately reports changes in patients' medical and emotional condition. Provides safe, individualized, quality care nursing to patients with varying medical/emotional needs.
