The Resident Assessment Coordinator (RAC) in the Community Living Center (CLC) is a professional registered nurse with a broad-based knowledge and application on the Resident Assessment Instrument Minimum Data Set (RAI-MDS) process according to the Center of Medicare and Medicaid Services (CMS) federal regulations and guidelines. The RAC has knowledge of current CMS Federal Regulations for Nursing Homes and its application to the RAI-MDS process. The Resident Assessment Coordinator (RAC) provides expertise and knowledge in promoting the application of the RAI-MDS assessment and the care planning process, considering the age specific needs of the residents. The RAC guides and supports the interdisciplinary treatment team (IDT) in completing the RAI-MDS assessment timely and accurately. He/she communicates to leadership and interdisciplinary team members any changes to VHA directives/handbooks and CMS guidelines related to the RAI-MDS process. The RAC functions as an expert consultant in identifying quality of care deficiencies; guiding committee's and teams in the appropriate plan of action and submits the plan in coordination with appropriate leadership. He/She directs the RN Care Coordinators who are responsible for MDS data entry for nursing. . Also, serves as a consultant to the facility, VISN, VACO and IDT who is responsible for their assigned sections for MDS data entry. He/she is effective in validating information, evaluating data to maximize Resource Utilization Groups (RUG) to provide the appropriate resources as it relates to Veterans Equitable Resource Allocation (VERA). The RAC serves as a resource and subject matter expert for RAI-MDS issues to improve process. He/she functions as escort to surveyors during the annual surveys. The RAC ensures RAI-MDS training for all new staff with initial and ongoing competency as needed. Maintains educational resources on RAI/MDS and others for long term care. He/she maintains mastery of technical skills needed to trouble shoot issues for RAI-MDS application, including monitoring of the Caribou and VISTA integration, Vista HL 7, and other related technical systems. He/she prepares appropriate reports utilizing the Quality Measures and the RAI-MDS Caribou application data for the Quality Assurance Performance Improvement Committee (PIC), local, VISN and national level. The RAC ensures all sequenced assessments and tracking forms are properly signed and completed before batching and submitting to the Austin data base. He/she reviews final validation report for records submitted with errors (warnings and/or rejected records) and makes corrections before resubmitting.
