The Northport VA is hiring one, full-time Staff RN to to function as a Care manager in their Home Based Primary Care department. Duties/Responsibilities include, but are not limited to: The HBPC RN functions as a Care manager and is responsible for delivering direct and indirect care for Veterans in the home setting. The Staff RN is responsible for developing and coordinating the patient's plan of care to include ongoing assessment, and reassessment, care planning, care coordination, monitoring and evaluating clinical outcomes. The Staff RN provides for the promotion and maintenance of health and prevention of illness and injury utilizing the nursing process. The Staff RN provides individualized education and support to the patient and caregiver to optimize clinical outcomes and support quality of life decisions. The Staff RN provides intensive care management linking Veterans with resources within and outside the VA and coordinating comprehensive care with VA and community providers. The Staff RN is also responsible and accountable for maintaining training required and may be assigned additional duties as area needs require. The Staff RN is expected to communicate timely and effectively with all members of the patient care team, as well as relate courteously and appropriately to all internal and external customers. The Staff RN is expected to work in a collaborative, team-oriented environment, and demonstrate consistent effective communication skills and exemplary professional behaviors. Fostering positive interdisciplinary collaborative relationships is essential for continuity of care that is safe and quality driven Nursing practice has an emphasis on health promotion, accountability, assessment, education, infection control, safety and psychological support. A multidisciplinary approach is used to assist the patient, family and/ or significant other( s) to attain optimal functioning ability. Measurable outcomes and nursing interventions are individualized. The Staff RN functions within a nursing process framework for problem-solving and care facilitation: assessment, planning, implementation, and evaluation. Job duties are focused on disease prevention and achieving optimum mental and physical health and well-being.
