The Licensed Practical Nurse GS-7 MOVE! Health and Wellness Coach working in the Medical Center and/or the Community-Based Outpatient Clinics (CBOC's) in VA Puget Sound supports and demonstrates an exceptional degree of practical nursing knowledge and skills necessary for concentrated effort and self-direction to provide care for the Veteran population The MOVE! LPN/LVN Health and Wellness Coach works closely with Veterans, family members, clinicians, and PACT members to assist Veterans seeking self-directed, lasting changes aligned with their values. 2) Champions patient-centered care principles and strategies to empower Veterans to make choices that will optimize health and well-being. 3) Collaborates with Veterans and Veteran's primary care and specialty care treatment providers to develop goals designed to improve health outcomes, reduce risk for chronic illness, and lessen the likelihood of admission to ED, inpatient care, or long-term care. 4) Effectively incorporates knowledge and understanding of established customer service standards in all interactions with patients, family members, and/or other internal/external customers. 5) Manages MOVE! Program consults by screening, receiving and alerting MOVE! Program Coordinator of consults that need further evaluation. 6) Provides education to Veterans, family and staff on MOVE! Program and options for participation. 7) Leads MOVE! orientation groups either in-person, VVC or on telephone as well as provide one on one Health and Wellness Coaching. 8) Enrolls Veterans into the MOVE! weight management program. 9) Fields telephone calls regarding the MOVE! Program and processes all requests for enrollment, primarily for the assigned site and as needed for other VA Puget Sound Health Care System sites. 10) Establishes constructive relationships with individual Veterans and their families to elicit feelings and attitudes, and to promote positive relationships, communication, and socialization skills. 11) Fosters an environment of respect for individual Veteran and family rights to privacy and dignity in all aspects of care delivery. 12) Displays unconditional positive regard for Veterans and a belief in their capacity for changes, and honor that each Veteran is an expert on their life while ensuring that all interactions are respectful and non-judgmental. 13) Develops a coaching/mentoring relationship with Veteran. 14) Assists Veterans through a process of actively working toward better health by providing support, encouragement, and education.
