The Home Based Primary Care (HBPC) mission is to provide comprehensive, interdisciplinary, primary care in the homes of veterans with complex medical, social and behavioral conditions for whom routine clinic-based care is not effective. The primary focus of HBPC is longitudinal care for complex, chronic, disabling disease. The HBPC Nurse, is a registered nurse who provides nursing in the home setting as a proficient clinician. Duties include, but not limited to: The HBPC Nurse serves as a member of an interdisciplinary team, and is integrally involved in the assessment, planning, intervention, evaluation and revision of a plan of care for patients, integrating professional nursing assessment, appropriate technologies and communicating with appropriate personnel. Providing care for patients based on knowledge of medical center, community and nursing home resources available to meet the identified needs. Providing clinical case management for each veteran. Developing and managing a home and community based plan of care which will assure quality of care, access to care and cost efficiency regardless of payer source. Providing appropriate home care for veterans ranging in age from 18 through the geriatric years, taking into consideration diagnosis, and physical, psychosocial, spiritual and cultural needs. Maintaining all required licensures, certifications and credentialing requirements, including maintaining and providing a copy of current Basic Life Support (BLS) certification. Work Schedule: Monday- Friday 0800-1630; Option for compressed tours.
