| The Float LPN reports to the unit Nurse Manager. The LPN delivers fundamental, knowledge-based care to assigned patients while developing technical competencies. The primary purpose of this position is to perform a very wide range of patient personal care, support diagnostic services, and chart activities on a regular and continual basis on a surgical, medical, rehabilitation, psychiatric, or long-term care ward or clinic. The incumbent performs duties in such a way as to promote a sense of worth and dignity within each individual. The Float LPN is responsible for the evidence-based care of a group of patients as follows: a. Assists the RN, charge LPN, and other members of the interdisciplinary team (IDT) with providing input in the development of care plan based on the Veterans care needs b. Administration of medications within the LPN scope of practice c. Under the direction of the RN or charge LPN, provides health care services to the Veteran to include, but not limited to: Foley Catheter insertion/discontinuation/care; obtaining the Veteran's vital signs, blood glucose, or EKG; activities of daily living (ADL); and post-mortem care. d. Takes appropriate action when indicated regarding equipment safety (Wanderguard unit, crash cart, refrigerators, etc.). Communicates to appropriate staff when immediate intervention is needed for safe care. Initiates and maintains standards of cleanliness and safety in Veteran environment and exercises proper use and maintenance of equipment. e. Monitors Veterans' risks for behaviors including, but not limited to: Elopement, wandering, suicide, fall, behavior, and pressure ulcer and taking action, when indicated, to include communicating to appropriate personnel, when further intervention is needed, and completing necessary documentation. Sits with patient on 1:1 for safety, as needed. f. Under the direction of the RN or charge LPN, monitor bladder and bowel functioning, tracking of input and output (I&O) and hydration. g. Performs nursing care, as applicable to the Veteran, including all aspects of personal care and grooming. h. Consistently follows policies, standard operating procedures (SOPs), and procedures as applicable to reusable medical equipment (RME) and infection control. i. Observes Veterans and reports changes in Veteran behavior and condition to the unit charge nurse. j. Reports required information for hand-off communication to charge nurse and co-workers including to other members of the IDT and other applicable staff and departments. Completes the necessary documentation for these duties. k. Under the direction of the RN or charge LPN, educates the Veterans on their care needs, as appropriate to LPN skill level. Communicates with families and significant others, as appropriate. l. Promptly reports any deviations from normal practice patterns to the charge nurse, NM, or the nursing supervisor. m. Completes all assigned routine documentation n. Responds during emergencies, assisting during fire drills, and responding to any safety issues as they arise. Demonstrates self-direction in applying knowledge of fire, safety, and disaster regulations, practices, and procedures, and communicates expectations to staff. o. Uses proper body mechanics and safe lifting equipment, including personal protective equipment (PPE), that promotes a safe workplace. Encourages other staff compliance with safety policies. p. Participates on committees or other workgroups as requested. Participates in quality improvement activities. q. Actively seeks educational opportunities to enhance nursing knowledge and skills. Completes mandatory education requirements. r. Ensuring the cleanliness of rooms by emptying trash, cleaning bedside tables, and ensuring that belongings are stored appropriately. s. Promotes the values of cultural transformation and Veteran-centered care to nursing staff in the Community Living Center (CLC). Actively seeks Veteran choice in care plan participation and goal development. t. Manage patient care by monitoring of psychiatric symptoms, assisting in medical and psychiatric emergencies, administering medications, assessing withdrawal symptoms, and providing appropriate, clinically sound care. u. Identifying and caring for suicidal Veterans and managing disturbed behavior through their specialized training v. One-to-One observation of patients for high risk of suicide, elopement, falls, etc. w. Additional duties as assigned. Work Schedule: 7:45am - 4:15pm, with weekend and Holiday Rotations Financial Disclosure Report: Not required Physical Standards: Heavy lifting, 45 pounds and over; pushing; reaching above shoulders; use of fingers; use of both hands required; walking 8 hours; standing 8 hours; repeated bending; climbing legs only; both legs required; near vision correctable at 13" to 16" to Jaeger 1 to 4; both eyes required; ability to distinguish basic colors; hearing (aid permitted); emotional stability; far vision correctable in one eye to 20/20 and 20/40 in the other. May be required to work with assaultive or combative patients.
