The Nurse would be responsible for care on Victory Lane, a new 40 employee 18-bed Long Term Care neighborhood which provides care for Veterans with multiple comorbidities. The leading diagnosis of the unit are Cerebral Vascular Accidents (CVA), hypertension, diabetes, dementia, and congestive heart failure. Our mission is to enable residents with multiple comorbidities to remain engaged in their environment at the level of their cognitive ability and to function at the highest level possible. The Direct Care/Clinical Nurse/Staff Nurse will fulfill a noble mission of caring for the Veterans that have sacrificed so much for our Country. This new Unit is part of the long term response to the COVID 19. Non-infected high risk Veterans are being relocated to the Carl Vinson VAMC for care. Through expect Nursing Care, the Community Living Center at Carl Vincen VAMC has remained COVID 19 free. Through continuation of expert Nursing Care, it is the intent of the Carl Vinson VAMC to keep it that way. These are permanent positions that were already planned, but the opening of the new Unit, Victory Lane, has taken on a new urgency because of the COVID-19 Pandemic. The Direct Care/Clinical Nurse/Staff Nurse will have a minimum of an Associate Degree in Nursing (ADN) or a Diploma in Nursing. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in until program level quality improvement processes and initiatives as well as customer service programs.
