This position is responsible for: The nurse navigator coordinates patient care discharge planning activities for all inpatients at Nemours Children's Hospital as directed by the Clinical Nurse Specialist ( CNS). .The nurse navigator works with all disciplines in planning for discharge, arranging for home health needs, transitioning to long term care facilities and coordinating durable medical equipment needs. The nurse navigator promotes the principles and practice of patient and family centered care.
Responsible for coordinating discharge planning beginning at admission including planning forpatient’s discharge and completing appropriate information and data to be communicated to all disciplines prior to and following discharge.
Assists with all aspects of navigating the patient through the health care process assuring that continuity of care is promoted in all patient care settings.
Participates in discharge planning rounds and family partnerships discussion when possible and as directed by the clinical nurse specialist (CNS) in conjunction with other disciplines.
Coordinates the discharge goals, discharge plans, interventions and patient and family discharge education with all disciplines as led by the CNS.
Communicates directly with the patient and family about the discharge plan
Documents all aspects of the discharge planning process in the discharge planning progress note in the electronic medical record.
Proactively communicate discharge needs and plans to the responsible physician(s) assuring mutual discharge goals.
Responsible for gathering data such as:medical records, laboratory information, x-rays and any other pertinent clinical information for discharge.
Collaborates with Kid’s Track manager in relation to ongoing needs for discharge follow-up
Arranges for patient transport needs for transfer to another facility ensuring pre-authorizationfor transport is requested and third party payor approval is obtained.
Responsible for communicating with any outside agencies in order to promote continuity of care across the continuum including those relating to DME/ home care arrangements.
Works closely with Utilization Management to identify patients at risk for denial of continued hospital services.
Verifies insurance and financial information for discharge needs and resource referral assuring that the family receives a list and makes a choice of potential outside agencies.
Responsible for making or delegating systematic discharge telephone follow-up calls within 48 hours of discharge.
Adheres to Nemours Standards of Behavior.
Graduate of accredited school of nursing, ASN Required, BSN preferred
American Heart Association BLS required
Minimum three years related clinical experience. Discharge planning preferred
Current Registered Nurse license in the State of Florida
