The Social Worker, as part of an interdisciplinary team, including the patient/family, physicians, nurses, therapists and payors ensures that the patients progress in the acute episode of care through post discharge and is quality driven while being efficient and cost effective.
The incumbent interacts with patients, family members, healthcare professionals, and community and state agencies in this effort. The Social Worker serves as a liaison between the hospital and community agencies or facilities for the exchange of clinical and referral information.
The Social Worker identifies high risk patients from a psychosocial/financial perspective, assesses the psychological needs of patients and families and provides information, support, counseling, care management, and referrals to appropriate resources.
The Social Worker works collaboratively with the RN Care Manager and care team to ensure patient needs are met and care delivery is coordinated across the continuum. The expertise of the Social Worker is sought to resolve psychosocial patient care issues and to develop and implement a complex patient transition/discharge plan. This professional ensures that patients are assisted to achieve their highest level of function and return to the least restrictive environment post hospitalization.
The incumbent performs duties and tasks in accordance with performance standards established for the job. The incumbent is responsible for participation in and completion of all patient safety initiatives appropriate to the position. In addition, the incumbent conducts all job responsibilities according to the Mission and Values of the organization.
Responsible for ensuring the very best patient/family experience by optimizing all customer interaction. Foster a positive work environment through mutual support and cooperation and adheres to GHS and unit specific Service Standards.
The incumbent reports to the Director of Care Management.

EDUCATION AND/OR EXPERIENCE:
Master's Degree in Social Work (MSW) from accredited university required.
Care Management certification is required after two years of hire.
A minimum of two (2) years of clinical experience as Social Worker in an acute care setting is preferred.
Experience in dealing with extended care facilities and community healthcare agencies.
A working knowledge of insurance coverage, various governmental programs, available community care agencies and long-term care facilities.
Knowledge of Social Work theories, therapies and techniques as used in individual family and group treatment.
