Assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs throughout the continuum in various settings through communication of available resources to promote quality cost-effective outcomes. The Clinical Case Manager provides discharge planning through assessment, referral and interdisciplinary planning for high risk population and also links people to systems that provide them with resources, services and opportunities to achieve and maintain maximum functioning.
Education: Associate’s Degree in Nursing with plans to obtain Bachelors Degree within 5 years.
Experience: Minimum of 3 years clinical experience. Previous experience in discharge planning, knowledge of clinical and social factors that affect patient’s functional status at discharge, knowledge of community resources, assessment skills.
Training: On the Job
License: Currently licensed as an RN in the State of Florida. Require Case Management Certification, including CCM, ACM or NACCM after 3 yrs. employment.
Other: Communication and interpersonal skills, basic computer, analytical, grammar and spelling skills. Ability to effectively multi-task responsibilities.
