Kaiser Permanente Washington is seeking to hire a Visiting Hospice Nurse to join our fantastic Eastside team! Our Eastside branch is based in Redmond and coverage would be for the Eastside in this fantastic position!
This is a 0.8 FTEevening shift position, generally 2pm - 10pm, (4) days per week,Monday - Friday, with one day off during the week. During orientation, work shift would generally be Monday - Friday, 8am to 5pm, with orientation period anticipated to last approximately (3) months.
Once oriented and on the evening shift, rotating weekends are involved among the team, with two additional weekdays off, of course, during your turn of rotation. Weekend rotation occurs approximately once every 4-6 weeks.
This is a fantastic opportunity for those attending school in the morning or those who have other needs or desires to have mornings off!
The Visiting Nurse Hospice will:
Part of an inter-disciplinary team, which may include RNs, Medical Social Workers, Spiritual Hospice Counselors, Physical or Occupational Therapists, Speech Pathologists, MDs, Home Health Aids and a caring Clinical Team Manager!
Provides comprehensive skilled nursing care and services to patients in the home setting
Works on an interdisciplinary team to provide care in the home health, palliative care and hospice programs
Accountable for the assessment, development of plan of care, education and consultation related to pain and symptom management as well as adjustment to changes in health status
Plans for the return of patient care to primary healthcare providers as indicated
Education Minimum: Bachelor's Degree in Nursing, or other applicable healthcare-related field. Preferred: Bachelors of Science in Nursing
Experience Minimum: 2 years of experience as a Registered Nurse. Prior experience in Medical Surgical or some form of Home Health or Hospice experience Preferred: 2 years of Home Health or Hospice experience
Knowledge/Skills Requirements Minimum: Intermediate ability to work independently. Ability to transfer / position patients as needs require. Basic laptop skills including MS Office Suite. Customer service skills. Demonstrated experience providing care planning within a community setting (continuum of care). Preferred: Basic or limitedMcKesson for homecare system experience.
License/Certifications Minimum: At time of hire Current Washington State RN License. Washington State driver-s license with proof of insurance.
